Our provider team is made of physicians, nurse practitioners, and physician assistants from a variety of backgrounds and specialties. Each one brings warmth, respect, and care to each of our patients in the comfort of their home.
Iris Health's medical assistants are another layer of support for our patients and their families. Our team will make sure you are prepared for your appointment by reviewing medical history, medications, and facilitating an exceptional experience. This skilled team will also be making things that much easier by doing labs in-home.
The Clinical Liaisons will meet you in the hospital to make sure you receive follow up care to ease the transition home after discharge. Our patients look forward to regular calls for chronic care management and remote patient monitoring to help reassure you're getting the care you need when you need it. Patients are more comfortable knowing someone is responding to their needs.
Our Patient Liaisons give some additional layers to your care. They are responsible for community outreach, patient advocacy, and networking to make sure our patients are connected to all of the services they need. You can find our Patient Liaisons out and about at community events, taking part in senior care committees, and aiding in your telehealth visits for virtual support.
